When denied suitable lodging at AFHT
You MUST call CMS and report that the lodging facility is not suitable and you are requesting a new lodging facility.
Take note of the date and time of the call and the name of the caller you spoke with.
Claim under "claim not found" on nonservice time claim page.
YOUR CLAIM:
Claim 130 mile basic day
For this claim, you will enter a non-service claim.
Reasons lodging is not suitable:
No Power - (No cooling or heating ability)
Room has not been "cleared and serviced"
Room does not include a bath or shower, wash basin, two chairs, and a writing table.
IF ANY OF THE THREE REASONS LODGING IS NOT SUITABLE LISTED ABOVE ARE APPLICABLE, REQUEST A NEW ROOM.

Claiming basic day account being forced to stay in unsuitable lodging in violation of Section 3 of the May 22, 1972 Memorandum of Understanding. Upon receiving my room I noticed it did not meet the minimum requirements for being suitable lodging because ________(reason)_________. I was denied a new room by hotel employee _________ at ____ hrs. At ____ hrs on __/__/__, I spoke to crew caller ________________ requesting a new lodging facility but was denied. Claiming basic day for being denied suitable lodging as outlined in Section 3 of the May 22, 1972 Memorandum of Understanding.
